We recognize that many clients may not visit the Center frequently. That’s why we’ll deploy a hybrid follow-up model that includes:

  • Community Health Promoter (CHP) follow-up at the household level
  • Mobile outreach visits and home-based wellness checks
  • Phone-based adherence reminders and teleconsultations
  • Family and caregiver involvement in care routines

This hybrid approach ensures no patient is left behind—even if they live far from formal health facilities.
